If you have had a text message inviting you to make an appointment for your planned health review or test, please contact the surgery reception team on 01307 463558. The appointment will be with the Practice nursing team.

Reception will advise you how long the appointment needs to be. They will also advise you if you will need to fast before the appointment, or if you need to provide a urine sample.

At the appointment, we will also ask you some lifestyle questions.

Once the doctor has reviewed your measurements and test results, we will share your results in the form of a health plan with some additional information. In some cases the doctor may suggest a follow up phone consultation with you or a further check. In other cases, this may not be required.

In addition, we are also interested to know if you are affected by any of these issues:

  • Do you find it difficult to understand information about your health or your treatments?
  • Do you have any problems with housing?
  • Do you have money problems that make it hard to meet your normal living needs?
  • Do you often feel lonely?

These issues are very personal and bringing these up is at your discretion.

If you would like to mention any of these to our health care assistant or nurse, they can arrange for someone in the healthcare team to support you with this.

If you wish to have a carer or relative accompany you to your appointment please bring them along.

On the day of your appointment if you have a high temperature or any other symptoms of winter illness please contact the surgery to rearrange.
